Adding a description to your questions isn't always necessary, as most of the time less words is better. But if you think your question needs clarification, a description can do the trick.

To add a description to your question:

Navigate to your Survey editor, and select the question you want to add a description to
On the survey panel on the right side of your screen, under the heading General > Description add your description and click Save changes
