Here's how your post-purchase survey works in Shopify POS:

Once you've completed a sale, scroll down to Apps
​

Click on the Loudhippo App card
Your survey will appear. Ask the customer your questions and record the responses. The responses will be recorded agains the sale in Shopify and LoudHippo.
