Survey response rate is calculated by taking the number of people who answered your survey divided by the number of people who saw it on your Order Status page. That number is then multiplied by 100 to give you a percentage.

Example response rate calculation:

Let’s say 20 people successfully completed checkout (and saw your survey on your Order Status page) and 15 completed it, then your response rate would be 75%, as (15/20) x 100 = 75%
