t's easy to add questions to your survey. You can either add questions from scratch (as shown below) or from the Question Library.
First decide what question type will work best for the data you want to gain. Read more on the 7 question types here.
Navigate to your Survey editor
A. You can click question type in from the toolbox on the left, or
B. Click the Add Question button
Then select the question type at the bottom left of the question box
Now you can edit your question:
Title: type out the question as the customer will read it
Description: An optional explanation of the question if necessary
Choices: If your question has predefined choices, provide all the options that you think will be important + Other. Read more on Choices here
